1) Station 2) Survey Name 3) Survey Type 4) Survey Status 5) Start Year 6) End Year 7) Survey ID Number (NOTE: system generated) 8 )Frequency of Survey 9) Resource Theme Level 1 10) Resource Theme Level 2 11) Environment 12) Primary Goal Source (Management Plan) 13) *Partners Level 1 (if Survey Type = Coop Inventory OR Coop Baseline Monitoring OR Coop Monitoring to Inform Management or Coop Research) 14) *Scientific Name of TESS Species (if TE question = YES) 15) *Biotic Group Level 1 (if Resource Theme Level 1 = Biological Integrity AND Non-listed OR TESS Species is NULL) essentially, this field must be user entered or derived from non-listed or TESS species entry 16) *Biotic Group Level 2 (if Resource Theme Level 1 = Biological Integrity AND Non-listed OR TESS Species is NULL) essentially, this field must be user entered or derived from a non-listed or TESS species entry
